【标题】 A Pilot Study of Continuous Transtracheal Mixed Venous Oxygen Saturation Monitoring

【摘要】In this study, we investigated the feasibility and the accuracy of transtracheal mixed venous oxygen saturation (Svo2) monitoring. Ten patients undergoing thoracic surgery were included in this study. A single-use pediatric pulse oximetry sensor was attached to the double-lumen tube between the tracheal and bronchial cuff. After anesthesia was induced, the double-lumen tube was inserted into the trachea and adjusted to the proper position. During surgery, the pulmonary arterial blood was sampled every 3 min for 15 min to measure the Svo2. The measurements made by the transtracheal pulmonary pulse oximeter (Sto2) were recorded at the same time that blood was sampled from the pulmonary artery for Svo2 measurements. The levels of measurement agreement between the Sto2 and the Svo2 were analyzed using the Bland and Altman method. The mean ±SD(range) oxygen saturation values during the data collecting period were 82.0%±4.9% (72%–91%) for the Sto2 and 82.2% ±5.5% (71%–91%) for the Svo2, respectively. The linear correlation coefficient of the regression analysis between the Sto2 and the Svo2 was 0.934 (P <0.05). A 95% confidence interval for absolute difference between the Sto2 and the Svo2 was 1.58%–2.09%. The mean±2 SD difference between the Sto2 and the Svo2 was 0.12%±3.97% on the Bland and Altman graph.Weconclude that it is feasible to monitor the pulmonary artery oxygen saturation continuously by a transtracheal pulse oximetry technique and that it can be done so accurately.

【摘要】Background: There is no information about the pharmacokinetic profile of propofol in Chinese children younger than 3 yr.This study was designed to determine a complete pharmacokinetic profile of a single dose of propofol in Chinese children of different ages.Methods: Arterial blood samples were obtained from 35 children with an American Society of Anesthesiologist physical status of I or II at 2, 4, 6, 8, 10, 20, 30, 45, 60, 90, 120, and 180 min after a single bolus intravenous injection of propofol (3 mg/kg).The plasma concentrations of propofol were measured using high-performance liquid chromatography with an ultraviolet detector. A population model was used to estimate the pharmacokinetics of propofol. Results: A three-compartment pharmacokinetic model best described the pharmacokinetics of propofol. Clearance was 0.185 l/min, the volume of distribution of the central compartment was 7.41 l, the peripheral volumes of distribution were 54.6 and 7.2 l, and the intercompartmental clearances were 0.614 and 0.692 l/min for a child of the average weight of 13.7 kg. The half-lives were 2.67, 14.89, and 310.60 min. Covariate models were applied, and weight was found to be significant covariate for the clearance and volume of distribution parameters.No significant age effect could be demonstrated on clearance or volume of distribution parameters after weight was taken into account.Conclusions: This study supports the case that the pharmacokinetic properties of propofol do not differ substantially across Chinese children of different ages after weight has been accounted for.

【摘要】Objective: Rosiglitazone, a potent agonist of peroxisome proliferator-activated receptor (PPAR)-, exerts anti-inflammatory effects in vitro and in vivo. This study was designated to determine the effects of rosiglitazone on endotoxin-induced acute lung injury in rats. Design: Prospective, experimental study. Setting: University research laboratory. Subjects: Thirty-six male Wistar rats.Interventions: All the animals were randomly assigned to one of six groups (n 6 per group) and were given either lipopolysaccharide (6 mg/kg intravenously) or saline, pretreated with rosiglitazone (0.3 mg/kg intravenously) or vehicle (10% dimethyl sulphoxide) 30 mins before lipopolysaccharide. The selective PPAR- antagonist GW9662 (0.3 mg/kg intravenously) or its vehicle (10% dimethyl sulphoxide) was given 20 mins before rosiglitazone. Measurements and Main Results: Endotoxemia for 4 hrs induced evident lung histologic injury and edema, both of which were significantly attenuated by rosiglitazone pretreatment. The protective effects of rosiglitazone were correlated with the reduction by 71% of the increase of myeloperoxidase activity and the reduction by 84% of the increase of malondialdehyde in the lung tissue. The pulmonary hyperproduction of nitric oxide was reduced by 82% of the increase related to lipopolysaccharide challenge Pretreatment with rosiglitazone also markedly suppressed lipopolysaccharide-induced expression of inducible nitric oxide synthase messenger RNA and protein in the lung, as demonstrated by reverse transcription-polymerase chain reaction or Western blot analysis. Immunohistochemical analysis revealed that rosiglitazone inhibited the formation of nitrotyrosine, a marker for peroxynitrite reactivity, in the lung tissue. In addition, the specific PPAR- antagonist GW9662 antagonized the effects of rosiglitazone.Conclusions: This study provides evidence, for the first time, thatthe PPAR- agonist rosiglitazone significantly reduces endotoxininduced acute lung injury in rats.
